I have read the Commercial Licensing clauses in my Multimedia Services Agreement and agree to abide by them. I have also been given ample opportunity to review the "Commercial Licensing Explained" Blog Post that is included with the Media Permissions Form I filled out prior to receiving the terms of this Commercial Licensing Addendum. I also acknowledge that I have the opportunity to ask additional questions or make comments about the Commercial Licensing of my multimedia content below. If I do not have any further questions, I hereby acknowledge that this Commercial Licensing Addendum becomes officially part of the Multimedia Services Agreement I have with Daniel R. Smarg and Dan Smarg Photography.
